Effects of 4Ulcercare on Leg Ulcer Recurrence and The Potential Cost Savings to The NHS Effects of 4Ulcercare on Leg Ulcer Recurrence and The Potential Cost Savings to The NHS
By Dr Nyjon Eccles BSc MBBS MRCP PhD

Contents Objectives
  1. To determine the effectiveness of 4Ulcercare in preventing recurrence of leg ulcers after healing and
  2. thereby to estimate any potential cost savings to the NHS on leg ulcer management.

Methods
289 randomly selected purchasers of 4Ulcercare were called and asked to complete a short questionnaire.

A questionnaire was designed to record time ulcer healing and rate of leg ulcer recurrence before and after use of the 4Ulcercare device.

Customers were contacted randomly from a list of purchasers on Magnopulse’s database. No incentives were offered to those surveyed. The only criteria for inclusion was whether the subject answered the telephone and whether they were willing to spend 5 minutes answering questions about their experience with the device.

Two third party assessors that were not employees of Magnopulse completed the questionnaires.

Our focus was on the 211 subjects that had ulcers that had already healed.

Results
Average age of subjects was 70.6 ± 0.63 years and sex distribution was 55:45 ratio female to male.

Mean duration of leg ulcers prior to using 4Ulcercare (96.2% on lower leg or ankle) was 4.4 ± 0.55 years.

65% (expected recurrence rate in the population of 67%) of those surveyed had ulcer recurrence (before using 4Ulcercare) with a mean number of recurrences of 2.41 times.

Mean duration of 4Ulcercare use (after ulcer healing) was 19.94 ± SE: 0.65 months and the device was being worn for an average of 15.85 ± SE: 0.57 hrs per day (recommended 8 hours overnight wear post ulcer healing).

Key Findings
  • All 211 users had healing of ulcers and mean time to heal was 3.57 ± SE: 0.20 months.
  • After wearing 4Ulcercare for an average of 19.94 months, none of the subjects had a recurrence of their leg ulcers whilst wearing the device (i.e. p< 10-4).
  • No subjects had ulcers that failed to heal or got worse whilst using the device.

We have estimated an unprecedented potential 51.2% reduction in spending by the NHS, which represents a reduction of at least £153.7 million per year (assuming an annual spend of £300 million per annum).

Conclusions
Subjects surveyed had ulcers for on average 4.4 years before use of 4Ulcercare but these ulcers healed within an average of 3.57 months after application of 4Ulcercare. After 19.94 months wearing 4Ulcercare as advised, 65% of those surveyed that had previously had a mean number of recurrences of 2.41, had no further ulcer recurrence.

ADVANTAGES TO THE USE OF 4ULCERCARE OVER THE CURRENT METHODS OF LEG ULCER TREATMENT:
  • An inexpensive and simple treatment modality that can be self-applied
  • More consistent healing of leg ulcers
  • More rapid healing of leg ulcers
  • Less need for nursing input due to ease of application of the device as well as more rapid healing and
  • A dramatic reduction in leg ulcer recurrence
  • A reduction in the need for preventative health promotion strategies
  • Elimination of the need to check for arterial blood supply compromise prior to treatment
